People of all ages may be able to pass life on through the gift of organ, eye and tissue donation. Medical conditions at the time of death determine what organs and tissues can be donated. Individuals should not rule themselves out when considering registering as an organ, eye and tissue donor. In New York State, persons 16 years of age or older may enroll in the New York State Donate Life Registry. See "Application Process" below to register to pass life on through the gift of organ donation.

Uninsured or underinsured. 300% or less of the poverty level.

Persons in need of medical and non-medical home care.

Sign language. Spanish. Russian. Ukrainian.

Six months of age or older.

New York State resident. U.S. citizenship is not required. Eligible if waiting for Medicaid or insurance coverage. Eligible to teen-agers if unable to use their parents' money or resources. Call for income guidelines which is the same for all components of the program.

Veteran of one of the US Armed Forces. Honorably discharged. Veterans of allied countries may be treated if care is authorized by said countries.

Other language assistance available.

Varies with program. Hospice care: must be terminally ill with six months or less to live. Hospice care provided in Monroe, Wayne, and Seneca Counties.
