Age 18 or older, developmentally disabled; HCBS Waiver eligible.

Ages 3 to 21 years. Referrals from public school due to multiple, complex disabilities: ages 3-21 years. OPWDD eligibility for residential services. Call for information.

Spanish. Sign language. Augmentative communication systems.

6 years of age and older. OPWDD eligibility. Co-occurring mental health or behavioral health concerns that is causing stress within the person's system of care.

Developmental disability, autism, cerebral palsy, epilepsy and other neurological impairments which constitute a lifelong mental or physical impairment that becomes apparent prior to age 22 years, is expected to continue indefinitely and constitutes a substantial handicap to a person's ability to function normally in society.

Individuals with serious health issues and​/or other social​/emotional situations including but not limited to: cancer, long-term or chronic illness, Downs Syndrome, Autism, Asperger's, at-risk youth, Anxiety Disorder, depression, OCD, ADD​/ADHD, Post Traumatic Stress Disorder, Reactive Detachment Disorder, learning disabilities, documented social or emotional debilitation, life transitions​/family Issues, trauma, abuse, neglect and​/or life-altering setbacks.
