Adult services: none. Children services: Ages 5-21 who are experiencing problems that interfere with their daily lives. Children must be accompanied by an adult during walk in hours.

Adults age 55 years and older with a substance abuse issue.

Gay, lesbian, bisexual, transgender, and questioning youth, up to age 25.

Transgender individuals ages 12-19.

Ages 50 years and older.

Visual impairment or legal blindness. Vocational​/employment services: For individuals who are legally blind, ages 14 and up, by referral of a New York State Commission for the Blind vocational rehabilitation counselor.

Senior citizens and individuals with disabilities who don't qualify to receive public assistance, Medicaid or EISEP.

Medicaid recipients, including those who are already members of a Managed Care Organization (individuals who have both Medicaid and Medicare are also eligible). Those eligible need to meet one of the following conditions: Two or more chronic health conditions, such as asthma, diabetes, heart disease, mental health condition, or substance use disorder A significant mental illness A diagnosis of HIV ​/ AIDS

NYS resident, who qualifies for Medicaid and is elderly, has a disability, or requires assistance with activities of daily living, or needs skilled care. Must be medically stable and capable of overseeing their own care or have a designated representative.
