Call for information.

General REOC entrance requirements include the following: Applicant must be 17 years or older and meet income and residency requirements. Entrance testing. Specific, additional programs requirements vary.

Any service member who was active, deployed or wounded in the last 7 years is eligible for assistance, with proper documentation. Please see attachment in resource for eligibility criteria.

Military families and veterans.

Ages 18 and older who are addicted to opiates such as heroin, codeine, hydrocodone, oxycontin and others.

Resident of area served.

Parent of family member of a child or adult with a developmental or physical disability.

-Be a U.S. Citizen or permanent resident of the U.S. -Have a confirmed diagnosis of a chronic or life-threatening disease OR need screening services related to symptoms of disease. -Be in active treatment, had treatment within the past 6 months, or going into treatment in the next 60 days (at a facility in the U.S. or U.S. Territories)

Anyone with chronic pain.

Birth to 21 years of age. Must be enrolled in Medicaid and have 2 chronic health conditions (HIV​/AIDS, Serious Emotional Disturbance or Complex Trauma, and other disabilities).
