Must be Medicaid eligible​/active Medicaid recipient and have: 2 chronic health conditions (asthma, diabetes, heart disease, COPD, obesity, substance abuse disorder, etc.) OR A single qualifying diagnosis, including serious mental illness (Bipolar Disorder, Schizophrenia, etc.), HIV​/AIDS, and significant behavioral, medical, or social risk conditions that can be addressed through care management.

Must have physical or multiple disabilities for most services. Preschool Special Education: age 3-5. Early Intervention Services: birth to age 3. Pediatric Rehabilitative Services: birth to age 21. Dental Services: age 3 and older. All other programs​/services: call for information.

Resident of area served, child​/youth under the age of 21, enrolled in Medicaid. The child​/youth must meet the NYS DOH eligibility. Significant behavioral, medical, or social risk factors which can be addressed through care management.

Residents of area served, ages 18 and older who are Medicaid eligible. Must be in need for long-term care services of 120-days or more per year.

Any service member who was active, deployed or wounded in the last 7 years is eligible for assistance, with proper documentation. Please see attachment in resource for eligibility criteria.
