Resident of area served, child​/youth under the age of 21, enrolled in Medicaid. The child​/youth must meet the NYS DOH eligibility. Significant behavioral, medical, or social risk factors which can be addressed through care management.

Medical care must be over a 2-hour drive to be eligible for free air transportation. The patient must also be ambulatory and medically stable as well as be able to walk and climb a few stairs to board a small aircraft.

Residents of area served, ages 18 and older who are Medicaid eligible. Must be in need for long-term care services of 120-days or more per year.

High school seniors in area served who identify as Black or African American and plan to continue schooling after graduation (2 or 4-year college, trade, technical, or professional school).

Any service member who was active, deployed or wounded in the last 7 years is eligible for assistance, with proper documentation. Please see attachment in resource for eligibility criteria.

Diagnosis of breast or gynecologic cancer.
