School Program: ages 5-21 years. Adult Training Program: ages 21+ years, no age cap and are Medicaid eligible. Community Habilitation: Medicaid eligible and by 23 years have one documented developmental disability.

Must have physical or multiple disabilities for most services. Preschool Special Education: ages 3-5 years. Early Intervention Services: ages birth-3 years. Pediatric Rehabilitative Services: ages birth-21 years. Dental Services: 3 years of age and older. All other programs​/services: call for information.

Livingston County resident receives priority placement. Medical substantiation of need. PRI screen. Signed admissions agreement.

Residents of Livingston County and surrounding areas.
Ages 18 years and older. Service provided per physician's orders. RN and social worker interviews on site or in home.

Residents of area served. Mental (adults over the age of 18 years, and adolescents between 13 and 17).
