Birth to five years old, as well as school-age children. Incoming referrals up to age 4.

Livingston County resident receives priority placement. Medical substantiation of need. PRI screen. Signed admissions agreement.

School Program: Ages 5-22 years. Adult Program: Ages 21 or older, no age cap and are Medicaid eligible.

American Sign Language

Residents of area served. Mental (adults over the age of 18 years, and adolescents between 13 and 17).

PRI done within last 90 days. Indicated need for long-term care and​/or rehabilitation. Recent medical assessment. Rehabilitation requires discharge potential to home or lower level of care.
