Navigator Program
thru: Community Health Care Association of New York State
• Assists individuals, families, and small businesses to enroll in the NY State Health Insurance Marketplace.
Language Access
Eligibility: Anyone under the age of 65.
Application Process: Walk in or call to make an appointment.
Required Documentation: Pay stubs or any proof of income. Identity paperwork if needed.
Fees: No fees for service.
Coverage Note: Ontario, Wayne, and Yates Counties.
