Application Process: Information taken during first visit: name, zip code, and ages of everyone in the home.
Required Documentation: None. Once a year attestation form required by New York state.
Fees: No fee for service.
Coverage Note: Primarily Ontario County. Also services Wayne, Seneca, Livingston and Monroe County.
