Lifeline Program

Eligibility: Must be income eligible to receive assistance through one of eight state programs: Medicaid, Home Relief, Supplemental Security Income, Aid to Families with Dependent Children, Home Energy Assistance Program, Food Stamps, Veterans Disability Pensions or Veterans Surviving Spouse Pensions.

Application Process: Complete application on website.

Required Documentation: Copy of benefit ID card. If individual only receives HEAP, a copy of HEAP approval notice, or a copy of utility or fuel bill which shows HEAP benefit.

Fees: No fee for services.

Coverage Note: Clifton Springs, Phelps, Interlaken, Ovid, and Trumansburg.
