Mental Health Services Children's SPOA
thru: Erie County Mental Health Department**
• Provides case consultation, systems navigation, referral, and linkage to mental health supports and services for children at risk of multiple hospitalizations and out-of-home treatment.
Eligibility: High risk children ages 5-24 (if enrolled by 21 years of age).
Application Process: Download the Children’s SPOA referral forms, available on website, and submit via email, fax, or mail.
Required Documentation: Call for information.
Fees: No fee for services.
