Memory Care and Enhanced Care - Cottage Grove

• Provides assisted living memory care in 3 homes each housing 12 Elders. Enhanced care is offered in a fourth home for persons living with advanced dementia and needing assistance with all aspects of daily life. Each residence is secure and Elders are able to enjoy time outdoors in the secure courtyard.
Eligibility: Need for memory care services and assistance with activities of daily living. DOH 3122 required prior to admission. Call for information.
