• Offers a day program for adults with physical, psychosocial, and medical needs. Encourages self-esteem, social interactions, and independence to keep the participants with their caregivers in the community.
- Social work.
- Activity programs.
- Assistance with daily medication.
- Personal care and hygiene.
- Nursing (on-site lab work, wound care, acute medical needs).
- Arrangement and transportation to and from medical appointments.
- Physical, speech, and occupational therapies.
- A hot noon meal that can accommodate special diets.
- Foot care.
• Transportation is provided to and from the program within Monroe County.
Eligibility: Adults with physical or psychosocial needs requiring day time support/supervision.
Application Process: Call for information, complete initial screening on the website.
Required Documentation: Insurance information, basic health information.
Fees: Private pay, Medicaid, Medicare.
