Assertive Community Treatment
thru: Evelyn Brandon Health Center
• Provides comprehensive, community-based program for individuals diagnosed with serious mental illness whose needs have not been met by traditional, office-based services.
• Provides services by team of clinicians, including:
- Mental health evaluation and treatment.
- Substance abuse evaluation and treatment.
- Employment services.
- Case management.
- Family psycho-education and support.
- Life-skills acquisition.
- Housing.
- Transition planning.
Eligibility: Resident of area served. Ages 18 and older with a diagnosis of major mental illness.
Application Process: All referrals are done through SPOA, https://www2.monroecounty.gov/mh-adult-spoa-referrals.

Fees: Medicaid.
