• Provides comprehensive developmental checks with referrals and follow-ups if needed. Screenings include:
- Vision
- Hearing
- Speech
- Dental
- Social and emotional competence
- Cognitive functioning
- Physical development
- Social determinants of health/education (including ACEs and Protective Factors)
Eligibility: Ages 1 to 5 years (cut off is 60 months).

Required Documentation: Consent form.
Fees: No fee for services.
Coverage Note: Chemung, Monroe, Steuben, Wayne, and Yates Counties.
