Eligibility: You or a dependent receive low income benefits under certain programs or if your total household income is below 135% of the federal poverty guidelines.

Application Process: Complete application available online and mail, fax or drop off to closest location.

Required Documentation: Call for information.

Fees: No fee for service.

Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca and Wayne Counties.
