Continuum of Care for Neuro Resource Facilitation
thru: Brain Injury Association of New York State
• Provides information and referral for individuals with brain injuries that includes:
- Statewide clearinghouse
- Resource assistance
- Support Groups
- Educational Programs
- Management Consultation
Eligibility: Brain Injury at age 22 or older and a New York State Resident.
Application Process: Call for information.
Required Documentation: None.
Fees: No fee for services.
