Eligibility: Age 18 or older with a mental health diagnosis.
Application Process: Call for an appointment.
Required Documentation: Proof of income and expenses for sliding scale.
Fees: Any insurance, Medicare, Medicaid, sliding scale based on income.
Coverage Note: Genesee, Livingston, Monroe, Ontario, Orleans, Wayne and Wyoming Counties.
Languages: - Staff provides in-person interpreter
