• Provides care coordination services to children and adults with intellectual and developmental disabilities. Care coordination combines developmental disability services and supports with health, wellness, behavioral health and family services.
Eligibility: Children and adults with intellectual and developmental disabilities. OPWDD eligible.

Required Documentation: Proof of disability.
Fees: Medicaid funded.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca and Wayne Counties.
