• Provides coordination of services for adults living with serious mental illness, substance abuse and medical concerns, offers referrals to:
- Residential Care Coordination.
- Assertive Community Treatment.
- AOT services through the SPOA process.
Language Access
Eligibility: Ages 18 and older. Residents of Chautauqua County.
Application Process: Fill out referral form on the website, or call.
Required Documentation: Call for information.
Fees: No fee for services.
