Chautauqua County Children's Single Point of Access (CSPOA )
thru: Chautauqua County Mental Hygiene Services
• Provides assesment and guidance for children, youth and families who are at risk of out-of-home placement.
- Referals to mental health and subtance abuse resources.
- Confidential coordination of community base service providers to assist the child and family. Child and family are welcome to attend the coordination meeting.
Language Access
Eligibility: Birth to age twenty-one. Residents of Chautauqua County.
Application Process: Fill out referral form on the website, or call.
Required Documentation: Call for information.
Fees: No fee for services.
