Comfort Care and Hospice Program

• Provides compassionate care and support for residents and families during the final stages of life. Focusing on maintaining comfort and honoring choices during the end-of-life experience. The 25-person Comfort Care neighborhood offers private and semi-private rooms. Services offered:

Holistic care
Short and long-term palliative care
On-site medical staff
High quality, round-the-clock skilled nursing care
Pain management expertise
Individual and family support and counseling
24-hour open visitation
Attention to family and spiritual issues
Recreation and therapy

Language Access

Eligibility: Skilled nursing level of care. Usually 3-12 month life expectancy. Accept non-curative, comfort care philosophy.

Application Process: Call for application.

Required Documentation: Call for information. Send PRI, Screen, medical summary and history. DNR (Do Not Resuscitate) defined. Statement accepting comfort care wishes.

Fees: Private pay. Approved Medicaid. Medicare. HMO's. Long term care insurance with approval.

Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca and Wayne Counties.
