• Provides an alternate living arrangement for those adults who have been deemed beyond curative care by their physician.

• Offers support for families of the terminally ill.

Eligibility: Ages 18 and older. Person unable to remain in own home. Physician's statement that person is in last 3 months of life. Patient's desire for no life prolonging measures.

Application Process: Call for evaluation. Most referrals via social workers and physicians.

Required Documentation: Call for information.

Fees: No fee for care but we ask for daily contribution, groceries and co-pays for medication.

Coverage Note: Wayne, Monroe and Ontario Counties.
