Hospice Services
thru: Journey Home, Inc.
• Offers hospice care for those that are terminally ill. Care includes two beds, 24 hour basic care provided by volunteers, and a R.N. available 24 hours by phone or in person.
Eligibility: Three months or less prognosis as written by a physician.
Application Process: Lifetime Care, Visiting Nurse Service, social worker, or family member.
Required Documentation: Call for information.
Fees: No fee for services. Client must be covered by Hospice certified agency, call for information.
