• Processes birth certificate replacements for homeless and at-risk individuals.
- For replacement Non-drivers ID, it must be a current and just lost or stolen ID and can not be expired.
Eligibility: Must be referred by a case manager using the proper referral form.
Application Process: Fill out and submit the online form: https://opendoormission.com/id4me. Clients will be assisted by appointment only. No walk-in clients will be accepted. All clients will be booked months out for an appointment, all others will be place on a wait list. If the client misses a scheduled appointment, they are not eligible to be placed on the wait list for 2 months. First come, first serve in reference to case management referrals.

Fees: No fees for services.
