thru: Person Centered Housing Options
• Helps clients get into affordable housing where they pay 30% of their income. This is a housing-first program. After acquiring housing, housing specialists continue working with clients as they transition into their new homes.
• Works with the community/landlords to connect. Housing is not readily available, must connect and engage with the program. PCHO does not have its own housing
Language Access
Eligibility: Must be homeless, meaning on the street, in a shelter, in a car, or abandoned building.

Required Documentation: Birth certificate, ID, and Social Security card. (PCHO can help clients obtain these documents.)
Fees: No fees for services.
Coverage Note: Monroe County.
