Home Health Care Services
• Provides health, personal hygiene, housekeeping, and other related supportive services for homebound individuals, as prescribed by a physician. Services include:
- Registered Nurses.
- Licensed Practical Nurses.
- Certified Home Health Aides.
- Personal Care Aides.
- Homemaker services.
• Offers the Consumer Directed Personal Assistance Program (CDPAP), which allows consumers to choose care providers, including family and friends.

Fees: Private Pay. Third Party Insurance. Medicaid.
