Intensive Case Management
thru: Elmira Psychiatric Center**
• Provides seriously and persistently mentally ill (SPMI) and seriously emotionally disturbed (SED) individuals who have been discharged from a residential or inpatient facility on-going treatment and supportive services including developing and maintaining viable living, working and social situations within the community.
Eligibility: Must have a referral from physician and be 18 or older.

Fees: Varies according to program.
Coverage Note: Allegany, Chemung, Ontario, Schuyler, Seneca, Steuben, Tompkins, Wayne, and Yates Counties.
