• Provides clinical services individually or in groups focusing on losing weight, improving nutrition, managing cholesterol, getting active, stopping smoking, and reducing stress.
- Providers include a nurse practitioner, tobacco cessation counselors, and dieticians.
Eligibility: Age 18 or older.
Application Process: Call, make an appointment, or visit website for more information. Referrals from PCP are encouraged.
Required Documentation: None. Medical records are assessed.
Fees: Most insurances accepted. Call for information.
Coverage Note: Monroe, Livingston, Ontario, Schuyler, Seneca, Wayne, Yates, Genesee, and Orleans Counties.
