• Provides outpatient treatment services for family members regardless of whether the substance user is in treatment.

• Provides dual diagnosis services (substances and mental health).

• Provides medication assisted treatment.

• Provides treatment for tobacco cessation.

• Provides access either in person or via telemedicine.

Eligibility: Persons with Substance Use Disorder and/or family member.

Required Documentation: Health insurance identification card and picture ID.

Fees: Commercial insurance, private pay. Not accepted: Medicaid. Sliding scale may be applied after evaluations have been paid. Call for information.
