Application Process: Complete the Application for Monroe County OASAS Residential Services - found at OASAS website or call for application.
Required Documentation: Call for information.
Fees: Active Department of Human Services case (Congregate Care Level II). SSI/SSD. Private Pay. No one denied admission based on inability to pay.
