Offer a variety of opportunities for consumers to work on personal goals in group settings with peers. Calendar of activities changes monthly and members choose their preferred activities. Activities include:
  • Cooking.
  • Volunteering.
  • Shopping.
  • Video and board games.
  • Card tournaments.
  • Computers.
  • Cognitive exercises/brain teasers.
  • Recreational outings.

 

Eligibility: 18 years and older and can show proof of brain injury or chronic health/medical issues.

Application Process: Call or email for information.

Required Documentation: Documentation of disability; Other requirements to be discussed in personal meeting.

Fees: Medicaid. Private Pay. Workman's Compensation. Third-Party Insurance, Managed Care (iCircle Care, Nascentia, and Fidelis).

Coverage Note: Monroe, Ontario, Livingston, Seneca, Wayne and Yates County.
