• Offers hospice care services for patients in who have made a conscious decision not to pursue curative treatment for a life-limiting illness.
Eligibility: Resident of area served. Must have an incurable illness with a prognosis of six months or less. Does Not wish to seek curative treatment. Must have a physician’s order for program services.

Required Documentation: Proof of income, physician's order.
