• Offers hospice care services for patients in Ontario and Yates Counties who have made a conscious decision not to pursue curative treatment for a life-limiting illness.

Eligibility: Must have an incurable illness with a prognosis of six months or less. Does Not wish to seek curative treatment. Must have a physician’s order for program services. Receive care in Ontario or Yates County.

Required Documentation: Proof of income, physician's order.

Fees: Free or reduced-fee services. Medicare, Medicaid, and select private insurance plans.
