• Helps take care of adults and gives them support.
• Helps arrange medical and other treatments can visit at home if needed.
• Assists with finding a place to live, public benefits, and getting other help they may need.

Eligibility: Adults with 2 or more chronic health conditions, or SMI, or SUD, or HIV/AIDS.
Application Process: Call for information.
Required Documentation: Medicaid card.
Fees: Medicaid pays for services, no fee for services to clients.
Coverage Note: Monroe County.
Cultural Competency: Cultural competency, diversity, equity, and inclusion training for staff
Languages: Spanish- Staff provides service in language offered, Language line
