• Offers access to housing and residential supports that are managed by OPWDD to ensure equity and availability of services to those in greatest need.

• Offers 24-hour staff supervised homes as well as a supportive apartment program.

Eligibility: Must be 18 years and older. Individuals with developmental disabilities who are OPWDD eligible.

Application Process: Call for information. Care Coordinator completes CRO application. A CRO application needs to be submitted to OPWDD before agency can consider placement.
