Children and Youth with Special Health Care Needs (CYSHCN)
thru: Monroe County Department of Public Health
• Provides a statewide, public health program that provides information and referral services for families of CYSHCN in obtaining resources for their children with special health care needs.
Eligibility: Birth to age 21 with a serious or chronic developmental condition.
Application Process: Call for information.
Required Documentation: None.
Fees: No fee for services.
