• Provides a Personalized Recovery Oriented Services (PROS) program that is designed to help overcome mental health challenges.

Eligibility: Ages 18 years and older. Axis I, II primary mental health diagnosis.

Application Process: Referrals accepted from licensed mental health providers and self referrals.

Required Documentation: Call for information.

Fees: Medicaid. Private pay: proof of income.

Coverage Note: Primarily Monroe County. No one will be turned away.
