• Provides assessment, counseling, and therapy for individuals with disabilities. including:
- Speech Therapy
- Physical Therapy
- Occupational Therapy
- Social Work
- Psychology
- Cognitive Assessments (Dementia/Geriatric) - Medical - Nursing - One time comprehensive psych evaluation
Eligibility: Individuals with developmental disabilities who have OPWDD eligibility.
Application Process: Call Central Intake Coordinator.
