Long Term Care Ombudsman Program

(Program)

* Mediates patient care issues in long-term care facilities trained, independent volunteers who are assigned to long-term care facilities. Ombudsmen listen, mediate, negotiate, propose and facilitate solutions to residents' concerns and problems.

Eligibility: Ombudsman volunteers: age 21 or over. Resident of any age at a skilled nursing or adult home.

Application Process: Call for information.

Required Documentation: Call for information.

Fees: No fee for service.

Coverage Note: Monroe, Genesee, Livingston, Ontario, Orleans, Seneca, Wayne, Wyoming and Yates Counties.

Cultural Competency: Signage and advertising for diverse populations, Cultural competency, diversity, equity, and inclusion training for staff

Languages: Language line, Materials/literature in language indicated
