• Provides a medical equipment loan closet available on a first come first served basis. Extendable three month limit on equipment loans:

Wheelchairs
Portable ramps
Walkers
Crutches
Canes
Bath items
Quad canes
Commodes, and raised toilet seats

• Offers Long-Term Care Programs and Services, Accesible and Affodable Housing Listings and application assistance, Assistive Equipment and Access Services, Benefits Support and Advocacy.

• Offers Consumer Directed Personal Assistance Program (CDPAP), Expanded In-Home Services for the Elderly Program (EISEP), Veteran Directed Care (VDC) Program, Traumatic Brain Injury (TBI) Waiver, nursing Home Transition and Diversion (NHTD) Waiver, Open Doord Program, Long-Term care Ombudsman Program (LTCOP), Housing Advocacy, Olmstead Housing Subsidy, Rapid Transition Housing (RTH).
