• Offers a 54 bed skilled nursing facility that provides various medical, mental, dental services and, rehabilitative services with a rehab unit that provides the following services:
- Colostomy
- Tube feedings
- Wound care
- Oxygen therapy
- Pain management
- Orthopedic post-surgical restorative care
- Short-term rehabilitation services
- Hospice services
Language Access
Eligibility: Must have a doctor's order for 24 hour skilled nursing care. Must have PRI.
Application Process: Call for information.

Fees: Medicare. Medicaid. Private pay. Third party insurance.
Coverage Note: Monroe, Livingston, Wayne, and Ontario Counties.
