Hospice Care

Eligibility: Have prognosis of 3 months or less to live. Receiving hospice care at home but need additional assistance. Be in the hospital without the possibility of going home.

Application Process: Referral by nurse, social worker, physician or care provider or local hospice agencies.

Required Documentation: None.

Fees: No fee for services.

Coverage Note: Monroe, Ontario, and Livingston Counties.
