Hospice Care
thru: Sunset House
• Provides palliative (comfort) care for those beyond curative or aggressive treatment. The resident maintains control and is assisted through this time of transition.
Eligibility: Life expectancy of 3 months or less.
Application Process: Referrals are taken through UR Medicine Home Care - Hospice Care or Lifetime Care Hospice.
Required Documentation: Call for information.
Fees: No fee for services. Donations accepted, but not required.
Coverage Note: Monroe County.
