Hospice Services
thru: Journey Home, Inc.
• Offers hospice care for those that are terminally ill.
- Two beds.
- 24 hour basic care needs provided by volunteers.
- R.N. is available on call 24 hours a day by phone or in person.
Eligibility: Three months or less prognosis as written by a physician.
Application Process: Lifetime Care, Visiting Nurse Service, social worker, or family member.
Required Documentation: Call for information.
Fees: No fee for services. Client must be covered by Hospice certifies agency, call for information.
Coverage Note: Monroe County.
