Eyeglasses E-Voucher Program
thru: New Eyes
• Provides e-vouchers for new prescription eyeglasses.
- Does not provide or pay for eye exams.
Eligibility: Income must be at or below 250% of the federal poverty level, have no other resources available to pay including federal, state, or charity programs, and have not received New Eyes voucher in the last 12 months.
Application Process: Application may be submitted by social worker, case work, agency on the website. Individuals may apply on this page if they do not have access to a social service agency: www.new-eyes.org/application
Required Documentation: Eyeglass prescription dated with the last 2 years.
Fees: $10 program fee for service.
