• Provides comprehensive developmental checks with referrals and follow-ups if needed. Screenings include:
- Vision.
- Hearing.
- Speech.
- Dental.
- Social and emotional competence.
- Cognitive functioning.
- Physical development.
- Social determinants of health/education (including ACEs and Protective Factors).
Eligibility: Ages 1 to 5 years (cut off is 60 months).

Required Documentation: Consent form.
Fees: No fee for services.
Coverage Note: Chemung, Monroe, Steuben, Wayne, and Yates Counties.
