• Provides support services for families. Services include:

Family Access and Connection Team(FACT) - engages with families of children and youth with high needs to set goals and work through challenges. Short term intervention including assessments and making referrals to service providers
Youth and Family Partnership(YFP) - Creates individualized plans that allow youth and family to develop strategies and supports to address and manage challenges
Family Voice - Assists families of children with mental, emotional, and behavioral problems. Family support includes individual support and Family Voice meetings

Eligibility: Resident of area served. Ages 5 to 21 varies on program.

Application Process: Call for information or visit website to complete on line referral form. Youth and Family Partnership: No referrals from general public. Referrals only accepted from SPOA, FACT, and Child Protective Services. +++++++++++++++++++++++++++++++++++++++++++++++++++++++++++++
