• Helps families navigate care by providing tele-health, essential resources, comfort items, and personalized support. The mobile unit visits clients homes.
Eligibility: Families of children 21 years and under receiving medical treatment thorugh URMC Pediatric Complex Care program.
Application Process: Referral from URMC Pediatric Complex Care program.
Required Documentation: Call for information.
Fees: No fee for services.
Coverage Note: Cayuga, Livingston, Monroe, Ontario, Seneca, and Wayne counties.
